Shredded Health & Performance Terms & Conditions
This contract is between Shredded and the afore-signed member. This contract relates to the terms and conditions of membership at Shredded. All members must also agree to the Shredded Gym Etiquette policies. All policies on this contract and the Shredded Gym Etiquette agreement are subject to change without prior notice. Any policy changes will be made available to members through Shredded notice boards located within the gym premises and are the responsibility of the signed member to adhere to at all times. Any breach of Shredded terms, conditions or policies may result in the immediate termination of the membership contract and loss of access to all Shredded Facilities.
Ezidebit I/We acknowledge that Ezidebit is acting as a Direct Debit Agent for the Business and that Ezidebit does not provide any goods or services (other than the direct debit collection services to me/us for the Business pursuant to the Direct Debit Request and this DDR Service Agreement) and has no express or implied liability in regards to the goods and services provided by the Business or the terms and conditions of any agreement that I/we have with the Business. I/We acknowledge that the debit amount will be debited from my/our account according to the terms and conditions of my/our agreement with the Business and the terms and conditions of the Direct Debit Request (and specifically the Debit Arrangement and the Fees/Charges detailed in the Direct Debit Request) and this DDR Service Agreement.
I/We acknowledge that bank account and/or credit card details have been verified against a recent bank statement to ensure accuracy of the details provided and I/we will contact my/our financial institution if I/we are uncertain of the accuracy of these details. I/We acknowledge that is my/our responsibility to ensure that there are sufficient cleared funds in the nominated account by the due date to enable the direct debit to be honoured on the debit date. Direct debits normally occur overnight, however transactions can take up to three (3) business days depending on the financial institution. Accordingly, I/we acknowledge and agree that sufficient funds will remain in the nominated account until the direct debit amount has been debited from the account and that if there are insufficient funds available, I/we agree that Ezidebit will not be held responsible for any fees and charges that may be charged by either my/our or its financial institution.

Membership Cards A membership ID card will be provided to members for identification purposes. If members visit the club without a valid access card, photo identification can be used. Failure to produce a membership card or valid photo-identification on arrival may result in refusal of entry. Shredded Membership cards are strictly for use by the membership holder only. If members are found to be sharing membership cards with non- members or other members, the involved parties will face an immediate stand-down period and a potential fine. The ID card is Shredded property and must be returned on termination of membership. There is a charge for lost ID cards of $15.00.
Payment of services All services with Shredded Directors are to be paid in full at the time of booking. Bookings will not be held without full payment. This includes but is not limited to monthly follow-ups, skinfolds, dry needling, competition consultation and other services. 45 minute training sessions with the directors are to be paid in full at reception prior to entry to the facilities.
Dishonor Fees $14.80 will be charged to your account when your payment is not successful. To avoid this please ensure you have sufficient funds at time of with drawl.
24 Hour Cancellation Policy If you need to cancel an appointment, we will require 24 hour’s notice. Failure to do so will result in you being charged for the full session.
Lockers Shredded provides lockers for your use on a daily basis only and suggests that you use a lock to protect your property. Do not leave any valuable property in a locker at any time. Shredded Pty Ltd is not responsible for any theft of or damage to your property. If you leave a lock on the locker overnight, Shredded has the right to remove it. Shredded will consider your property abandoned if left overnight. If you leave your property overnight, Shredded shall have the right to donate your property to charity.
Dress / Towel policy Shredded Health & Performance requires you to wear appropriate clothing and footwear while in the facilities. Here are general guidelines: gym shorts, T-shirts, jogging, loose track suits and compression garments are great for training, but street clothes/shoes and jeans are not. No street or blacksoled shoes permitted. You must have a cloth towel with you during workouts to protect and clean machines you use.
General Policies for Minors Shredded does not offer childcare facilities and as such, children are not to be left unattended anywhere within the Shredded Facilities. Children are not permitted to enter the training areas at any time. Minors Under 12: May not use the facilities at any time. Minors 12-17: May use the facilities if under the supervision of a coach at all times and their parent or guardian has signed the financial guarantee and the Membership Agreement. Shredded reserves the right, in its discretion, to require that a parent or guardian accompany a minor.
Use of member images By signing this contract, you agree to allow Shredded staff to take video or photos to use for promotional purposes. Staff will always seek permission first. If you are unhappy with something that is posted on social media or throughout the gym you may request to take the footage/image down within 7 days of posting. Once posted on social media Shredded reserves the rights to use such footage and images for further promotions to show people the services and results we have at Shredded.
Suspensions Suspension of membership is at a cost of $10 per week per suspension. The suspension form is to be completed and submitted to firstname.lastname@example.org. All Shredded members are entitled to a maximum of 2 weeks suspension of membership for every 12 weeks of contract. Extended suspension will be considered on a case by case basis and may require evidence to support extended suspension requests. All suspension applications must be provided 5 days ahead of suspension dates, this also ensures membership auto payments are correctly adjusted and processed on time
Cancellation Policy All memberships continue to run after the minimum agreed term unless specified. After the agreed term, members can terminate the contract with a termination form given by the Customer Care Manager. Terminations require a two (2) week minimum notice period. All terminations must be submitted on a termination form with no exceptions. All early terminations require 25% payment of the outstanding membership fees. If special circumstance arise, Shredded management will consider an early termination on a case by case basis.
Violation of Rules If any member, client or guest violates any of the Policies or Rules, Shredded will ask that person to stop or leave. A violation may also cause Shredded to terminate the violator's membership according to the terms of their Membership Agreement.
